Haven Hospice is the source for patients, their families, and their healthcare providers to find answers to their advanced illness challenges. They have provide services in 18 counties in North Florida since 1979, and have been licensed as a not-for-profit hospice since 1980.

Haven’s team of medical providers and staff expertly manage pain and symptoms at home or in a variety of settings including hospitals, nursing homes, or assisted living facilities. Their hospice patients benefit from the support of an interdisciplinary team: physicians and nurse practitioners, registered nurses, social workers, chaplains, hospice care aide(s) and volunteers.

Haven Hospice’s mission is to honor life by providing comfort, care, and compassion to individuals and the families they serve. Most hospice patient care is funded through Medicare, Medicaid or private insurance. If these sources prove to be unavailable, inadequate, insufficient or exhausted, then hospice care services at Haven are provided based upon need, not ability to pay. These one-on-one sessions are 45 minutes long and are performed in our 40-foot mobile unit that travels throughout Jacksonville.
